Client Forms

Upon entering our office, our receptionist will welcome you as a member of our family.  We will request that you complete our New Client Intake forms located in the New Client Center. This paperwork provides us with your health history, information about your current health status and your health goals.

Consultation

Next, you will have a consultation with your doctor to discuss your current health status. This initial visit is designed for our doctors to learn more about you, your expectations, and your current health goals to determine how chiropractic care can meet your needs.

Examination

After your consultation, with the help of our CA (Chiropractic Assistant), the doctor will perform a complete chiropractic examination including a computerized nerve system stress test. We'll also evaluate your posture, body balance and perform a computerized foot scan so we can assess your current structural challenges.  

X-Ray Studies

X-rays help us determine if there are structural challenges or damage to your spine, and are a necessary piece of a complete spinal exam. Once we know the condition of your spine, we can develop the most effective care plan for you.  Most people are amazed once they see their X-rays up close.  In some cases, they can even identify their misalignments and degeneration themselves.

Report of Findings

Once the information is collected and all evaluations are performed your first visit will be completed.  You will then be scheduled for your second visit called your Report Of Findings (ROF).  The time between visits allows your doctor to review your collected information: your health history, goals, the results from examining your spine and X-rays, and prepare a detailed report for you.  The very next day is ideal for your ROF and we recommend that, if married, you bring your spouse to hear what the doctor has to tell you.  We'll answer all your questions during your ROF, including:

1. Do I have any challenges to my optimal health?
2. Can you help me?
3. How long will it take?
4. What will my care cost?

 Your doctor will discuss recommendations and notify you if your condition requires care with other providers.  Our doctors will  provide the best corrective care plan and wellness program to fit your needs.

Adjustments

 At the conclusion of your Report of Findings appointment, you will experience your first adjustment. This will include personalized spinal adjustments.  The doctor will determine what type of adjustment is best suited for your spine and nerve system.  The care you receive is interactive, so please communicate any questions or concerns regarding your care to your doctor at any time.

Home Care

How you participate in your healing process is just as important as the care you will receive in our office.  Your being fully engaged in your care program will bring you faster results at less cost.  As part of your care plan, we will provide you with home care; things that you will need to do when you're not in the office.  If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches.  If you desire, our wellness team will work with you to create healthy habits and routines for your lifestyle.  After all, achieving health and wellness comes from making good lifestyle choices.  Every person is unique, therefore everyone requires a customized wellness plan.  The purpose of our wellness program is for you to achieve good spinal alignment, have a healthy diet, an exercise program suited to you, taking time to be well rested, and maintain a positive mental state.  If you'll let us coach you through this process and you play full out, you'll live the quality of life that you desire for the rest of your life.

Special Consultation & Wellness Workshops

We ask all of our new clients to attend a special consultation, in fact we think it's so important that you attend we've included it in the cost of your initial visit to our office.  We only ask that you attend this special consultation once, and those that do consistently get well faster for less money.  Those that don't, tend to not get the results they desire or it takes them longer and costs more.  We hold these special consultations several times per month on alternating Mondays at 12:30 pm with Dr. Sienes, Tuesdays at 6:30 pm with Dr. Cockrell and Wednesdays at 6:30 pm with Dr. Mathre.  Please refer to the calendar for the next available date and time, but please call us at 223-2225 with your chosen day so that we can confirm a seat for you.  Friends and family always welcome.

We periodically hold workshops with other providers as the presenters, or specific topics that we think will be of intererst to you.  We are very willing to present on topics that you find interesting, so if you would like more information on a certain subject, please bring it to our attention.  Watch the calendar on the website for those special workshops as well as our other 'goings on' that we'll post on the calendar. 

Belong to a community group or need speakers at your workplace?  We have an entire "Menu" of talks that we will be happy to share with you and your group; topics relating to health and wellness as well as growing healthy kids or living to 100.  Speak to our CA's or one of the doctors, we're happy to share any information we can with you and your group.
